What is endometriosis?

😖 Endometriosis is a chronic disease affecting around one in ten women of childbearing age. It is characterized by the presence of endometrial tissue, which normally lines the inside of the uterus, outside it. These endometrial implants can be found on the ovaries, fallopian tubes, peritoneum, and sometimes even on other pelvic or abdominal organs.

This endometrial tissue outside the uterus reacts to menstrual hormones in the same way as the endometrium inside the uterus: it thickens, breaks down and bleeds with each menstrual cycle. However, unlike normal endometrial tissue, this blood has nowhere to go. This can lead to inflammation, cysts, scarring and adhesions, where pelvic tissue can weld together.

Why does endometriosis hurt so much?

👉 The pain associated with endometriosis can be particularly intense and disabling. Several mechanisms explain this pain:

Inflammation

🔥 Endometriosis causes chronic inflammation in the pelvic region. Endometrial cells outside the uterus release inflammatory chemicals, causing irritation and pain to surrounding tissues.

Adhesions

Adhesions are bands of scar tissue that can form between pelvic organs, binding them together. Adhesions can cause pelvic pain, bowel problems and fertility issues.

Ovarian cysts

👉 Endometrial implants on the ovaries can form cysts called endometriomas or "chocolate". These cysts can become quite large and painful, especially during menstruation.

This is the most widely accepted theory. It suggests that during menstruation, some of the menstrual blood, which contains endometrial cells, travels up the fallopian tubes and deposits in the pelvic cavity. These cells then attach themselves to the pelvic organs and begin to grow.

Dissemination via the lymphatic or blood system

👉 Endometrial cells could move to other parts of the body via the lymphatic or blood system, in a similar way to the spread of cancer cells.

Genetic factors

Endometriosis tends to run in families. Women who have relatives (mothers, sisters) with endometriosis are more likely to develop the disease themselves.

Immune disorders

Some research suggests that immune system abnormalities may prevent the body from recognizing and destroying endometrial tissue outside the uterus.
